We are encouraging patients to give their views about the services we provide. We would like to be able to find out the opinions of as many patients as possible and we are asking patients if they would like to provide their email address so that we can contact you by email every now and then to ask you a few questions. If you are interested please fill out the form below and submit it. We will then contact you with a password which will allow you to take part in the group. Your contact details will only be used for this purpose and will be kept safely.
