I need to register with the practice

REGISTRATION

Please call at the reception desk to fill in registration forms. We will need you to make an appointment with the Nurse for a routine health check and cannot fully register you until then. The receptionist does not always know where every street is, and may have to ask the Practice Manager if you are in our catchment area.

It may take some time for your medical records to come to us from your previous doctor. If you have any ongoing medical problems, or are on any repeat medications, please make an appointment with the doctor to discuss these. We will not be able to help you with repeat prescriptions until the doctor has authorised them.

If you have just moved into the area, we should be able to help you. However since there has been a lot of new building in the area, and just outside our boundaries, we have had to be quite strict in keeping to our geographical limits. This has been necessary to avoid disadvantage to the patients who live within our boundaries, and because we already cover a wide area including Scholes and Barwick in Elmet. We are sorry for any inconvenience which might be caused by this policy, but would like to remind you that there are a number of practices in the area with overlapping boundaries, so there is always a choice available.

Also, and for the same reasons, we are unable to accommodate patients who are already on the list of another local doctor, except where other members of the household are already registered with us, or where you are outside the boundaries of your present doctor, but within ours.

NEW PATIENT CHECK UP (registration check)

We encourage all new patients to have a registration medical check with the practice nurse. Existing patients are also welcome to come for a medical. The check is completely free and takes about 20 minutes. The purpose of the check is to find out about any existing problems for which you are receiving treatment, and also, to help you live a healthy lifestyle. The nurse will ask about:

Current and past illnesses and operations

-Illnesses that run in the family
-Medication and allergies
-Any screening tests such as cervical smears
-Any immunisations such as tetanus
-She will check your blood pressure, weight and height and test a sample of your urine.

You are also offered advice on:

-Healthy eating
-Exercise
-Sensible limits for alcohol
-How to stop smoking
-She will assess your risk of heart disease and can arrange a cholesterol check if appropriate.
